By becoming a member you will receive discounted rates on registration fees for sports and rec programs, as well as PLYSA events. You will also receive any of the other priveliges, as listed below, that come with each level of membership.

To be billed later, please fill out the form below. You will then be billed for the amount you have requested.

If you would like to pay by credit card by using PayPal secure online payment, please click on the button next to the level of membership you prefer.

NOTE: To complete the process you must:

1. Complete and submit the registration form.

2. Return to this page and pay.

Please Note: You will be charged a convenience fee of 4% of the registration costs. This is to help cover the credit card processing fees. To avoid this fee you can choose to be billed later.

Platinum Member ($1,000): Platinum membership will entitle a family* to lifetime benefits that will include an annual letter of thanks, a wall plaque, a vinyl sign with Sponsors name and a mention in each newsletter.

Lifetime Membership (Individual - $100.00): Individual Lifetime Membership gives an individual person lifetime benefits.

Lifetime Membership (Family* - $250.00): Family* lifetime membership will give you and your spouse lifetime benefits, and all children will receive the same benefits until the age of 18.

Family* Membership ($25.00): This will entitle the whole family* to membership benefits. Renewed annually.

Individual Membership ($10.00) : This will entitle an individual to all membership benefits for one year. Renewable annually.

*A family is defined as up to two adults and any children under the age of 18.
